Why Forms?
You have a powerful Form Manager tool included in your Giving platform. While you’re already using a form to collect donations, did you know you can also create an unlimited number of other forms?
From prayer requests to volunteer sign-ups to event registrations—you have access to nine customizable templates for the most commonly created forms. And you’re not limited to those! You can build almost any form your ministry dreams up.

Where to Access Forms
To access the Form Manager:
From your Manager Dashboard, click the Forms tab in the left-hand menu.
From there, you’ll see a list of all current forms, submission stats, and options to create or edit forms.
Create a New Form
There are three ways to create a new form:
- Start from a Template
- Copy an existing form
- Build completely from scratch
- Each method gives you control over layout, field types, payment options, and design elements.
