Dedicated Form mode locks the Kiosk to a single form experience—ideal when you want to focus users on one registration, signup, or payment form. Since the form itself manages its fund and fee settings, this mode requires the simplest configuration on the Kiosk side.
When to Use Dedicated Form
The Dedicated Form mode is perfect if you want your users to fill out a specific form. That form is the only thing they will be able to access. The form can be any type — for example, giving, event registration, contact, etc.
Dedicated Form Settings

From the Mode Selection page, tap Dedicated Form.

The currently selected Dedicated Form settings are displayed. If the settings need to be adjusted, tap Edit Settings. Otherwise, tap Launch Dedicated Form.
- Available settings:
- Location: If your organization uses the Location feature in the Giving Admin portal, you can set the location linked to your transactions. If you do not need the transactions linked to a particular Location, you may select the None option from the Locations list.
- Card Reader: This shows whether the card reader is connected. If the form you are using for this session requires payment, the card reader must be connected. If there is no payment associated with the form, feel free to leave the card reader disconnected.

Once you tap Launch Dedicated Form, you will see a list of available forms. You can scroll through the list to find the form, filter by form type, or search by name. Tap on the form to select it.
- You will see a preview of the form. Tap Lock Form.
User Experience
- When the user approaches the Kiosk, they will see your selected form:
- They will fill out the form and tap Next.

If the form requires no payment, the user will see a Submissions Successful message

If the form requires payment, using the card reader, the donor Inserts, Swipes, or Taps their card.

After payment completes, the donor can tap Continue to enter their contact information for the Receipt or tap No Thanks to return to the form.

If the donor wants a receipt, they must enter their First Name, Last Name, and Email Address. Phone Number is optional. The donor taps Send Receipt.

The donor sees a Receipt Sent message and taps done.
