The General Info section of Form Properties allows you to:

Rename your form

Add a description for clarity and engagement

Download a shareable QR code

Access the form’s direct link and embed code

Set time zones for submissions

Categorize your form for integrations and manager access
✏️ Form Name
If your form was created using a template, it likely has a generic name. Rename your form here to reflect your event, campaign, or fundraiser for clarity.
📝 Description
This area is perfect for giving context to your form, especially if it’s shared outside of your website.
Use it to:

Describe your event or cause

Explain fees or next steps
-
Share your mission or contact info
✅ Field Validation
By default, all fields are optional.
You can use this setting to:

Mark specific fields as required
-
Choose whether required/optional indicators show on the form
Each individual field can also be set to “required” via its own properties.
📱 QR Code
A unique QR code is generated for every form.
Download and place it on:

Posters
-
Slides
-
Event signage
-
Printed materials
Great for in-person events or announcements!
💻 Embed Code
Embed your form directly on your website.
Benefits include:

Enhancing trust with your branding

Streamlining the giving or registration experience
📌 Tip: Send the embed code to your website admin. You can say:
“Please embed this form like you would a YouTube video.”
🔒 Security Note:
Embedded forms must be placed on a secure (https) webpage with SSL encryption. Contact your web provider if SSL isn’t enabled yet.

🔁 After form submission, redirect users back to your site using the Submission tab in Form Properties.
🌍 Time Zone
The form’s time zone affects open and close dates for submissions (e.g., event registration deadlines).
Set or adjust the time zone here before launching your form.
🗂️ Categories
Categories influence how your form behaves and where it appears:

Manager Permissions – Controls which managers can access the form

Static Verbiage – e.g., “Make this gift recurring” vs. “Make this payment recurring”

MinistryOne Integration – Dictates where your form appears (e.g., Contact Module, Events Module)
